1. Who we are

Colorado Hormones is operated by MNP Hormones PLLC (“Colorado Hormones,” “we,” “us”). We provide telehealth hormone care to residents of Colorado. Clinical care is provided by Marnie Predovich, ACNPC-AG, a licensed nurse practitioner. By using this website or becoming a patient, you agree to these terms.

2. Telehealth services

Your first visit can be in person at your home or office, and follow-up visits are conducted by secure video. Lab draws are arranged with a mobile phlebotomist at your home or office, and prescribed medications are compounded by a licensed pharmacy and shipped to you. Our services are available only to people physically located in Colorado at the time of each visit. Telehealth may not be appropriate for every condition, and your provider may recommend in-person care when clinically needed. Telehealth care is not suitable for emergencies. If you think you are having a medical emergency, call 911.

3. Eligibility and the free welcome call

The welcome call is free and is not a medical visit. If you enroll, your initial labs and provider review are part of the 4-Month Reset. Starting a treatment protocol is a clinical decision, and we may decline to treat if labs or history show hormone therapy is not appropriate for you. In that case we refund your program fee in full.

4. Fees and payment

• 4-Month Reset (months 1 to 4): $2,400, paid in full at enrollment, and includes your initial labs and provider review. Monthly financing may be available through Cherry, a third-party lender. Cherry’s own terms, rates, and credit approval apply, and we are not a party to your financing agreement.
• Ongoing Care (month 5 onward): $199 per month, billed automatically each month to the card you provide until you cancel. There is no long-term contract.
• Medication costs are separate from program fees. Lab monitoring and phlebotomy are included in the program fees described on our pricing section.
• By enrolling you authorize us to store your payment method securely with our payment processor and to charge it for the amounts above. We do not store full card numbers on this website.

5. Refunds

• Welcome call: no charge, nothing to refund.
• 4-Month Reset: if you ask for a refund in writing before your provider has prescribed a treatment protocol, we will refund the full program fee within 10 business days.
• Once a protocol has been prescribed, the program fee is non-refundable, because clinical time, lab review, and medication coordination have begun, except where Colorado law requires otherwise.
• Ongoing Care: monthly fees are not refunded for a month already billed. See cancellation below.
• Medication purchases are handled by the dispensing pharmacy and are subject to the pharmacy’s own policies.

6. Cancelling Ongoing Care

You can cancel Ongoing Care at any time by emailing dan@coloradohormones.com from the email address on your account, or by calling 720-504-6488. Cancellation takes effect before your next billing date if we receive your request at least 3 business days before it. There are no cancellation fees. Please talk with your provider before stopping hormone therapy, because stopping suddenly may affect how you feel.

7. Billing questions and disputes

If you see a charge you do not recognize or disagree with, please contact us first at the email or phone number below. We will respond within 2 business days and work to resolve it quickly.

8. No guarantee of results

Individual results vary. Nothing on this website is a promise of a specific outcome. Information on this website is educational and is not medical advice. Medical advice is given only by your provider during a visit, based on your history, exam, and labs.

9. Compounded medications

Compounded medications are prepared by a licensed pharmacy for an individual patient. They are not FDA-approved and are not reviewed by the FDA for safety, effectiveness, or quality. Your provider will discuss this with you before prescribing.

10. Communications

We may contact you by email, phone, or text about your care, scheduling, and billing. Please do not send urgent medical concerns by email or portal message, since they may not be read right away. For emergencies call 911.

11. Website use

Content on this website belongs to MNP Hormones PLLC and may not be copied or reused without permission. We try to keep information accurate but may update pricing, services, or these terms at any time. The version on this page applies when you enroll.

12. Governing law

These terms are governed by the laws of the State of Colorado.

13. Promotional and partner offers

From time to time we offer promotions, including offers for clients of partner businesses. The terms of a promotion are stated on the page where it is offered and apply in addition to these terms. For the Castle Rock Contouring client offer, which ends 12/31/26:

• Free labs are available to those who qualify on the 15-minute welcome call.
• The 4-Month Reset program fee ($2,400) is waived for people who accept the offer.
• Card on file: we ask for a payment card before your labs. We do not charge it for the free labs or the waived Reset fee.
• Missed results visit fee: you can reschedule your free results visit with Marnie once at no charge. After that, a $200 fee is charged to your card on file if you miss the visit.
• Under this offer, your $199 per month membership begins right away instead of after a 4-Month Reset, and you can cancel anytime as described in section 6. Medications are billed separately.

If you do not accept the offer or do not qualify, no fee applies and your card is not charged.
